Valor Wrestling Academy (hereinafter referred to as “Academy”) is a wrestling program of Anthem Church (hereinafter referred to as “Church”). The Academy’s central location is at Anthem Church whose address is at 455 W. Depot St., Angier, NC 27501. The purpose of this program is to train boys from age 10 to 17 in scripture and wrestling. Every volunteer for the Academy has had a background check.
I understand wrestling is a one on one combat and grappling sport and participation in this sport has an inherent risk of injury and, although highly unlikely, the potential of death. Training for the Academy will typically take place at the Anthem Church building two Saturdays per month from 9:00 am to noon or 9:00 am to 3:00 pm unless otherwise notified (times are determined by the Academy coaches). A few times a year the Academy will participate in wrestling tournaments. There will also be a camp during the summer.
I am not aware of any injury, illness, or other health related issue(s) which would restrict or limit my child’s ability to play or otherwise engage in wrestling at the Academy.
I agree to assume any and all risks, liabilities, and/or expenses due to any injury that may occur as a result of my child’s involvement in the Academy and any wrestling associated activities with the Academy.
I agree to hereby expressly release, hold harmless, and covenant not to sue the Academy, Church, and/or any person operating on their behalf as a coach, coaching assistant, administrator, or other associated volunteer for the Academy from any injury, illness, or any other physical harm to my child which occurs during, or arises out of, the participation of my child or otherwise their engagement in any activity detailed herein while under the care and supervision of the above persons or locations.
