EMPLOYMENT APPLICATION

I authorize inquiries of any person or organization to give or verify information pertinent for employment with Palmetto Dining Group, LLC and/or Beaches & Cream, Inc. "Palmetto Dining Group". I understand and agree that this application and other submissions become the property of Palmetto Dining Group and that any false or misleading statements or answers on this application form or any supplements thereto or in any interviews may result in cancellation of my application or in immediate dismissal if subsequently employed. Furthermore, I agree that Palmetto Dining Group shall not be liable in any respect if I am not hired or if my employment is terminated as a result of providing such false, misleading or incomplete information. I agree to take a physical examination and/or drug test, if required; to return any property upon termination of my employment; and to comply with all the rules, regulations, policies, and procedures of Palmetto Dining Group. In consideration for employment, I hereby agree to be bound by Palmetto Dining Group's policies and procedures as set forth in the employee handbook, as it may be adopted from time to time. I hereby acknowledge that a copy of the handbook is available for my review.
