This contract hereby provides the release of liability of Forget-Me-Not Animal Rescue (FMN) organization and its volunteers due to the actions of any rescue pet which has been released into the ownership of FMN and place into a Foster Home

I, the Foster Care provider named above, understand that FMN and its volunteers have no way to guarantee the actions or disposition of any pet released to FMN and place into Foster Care with me. I understand that being living animals, pets may act unpredictably in any given situation and that I must take necessary precautions to prevent accidents, injuries, and property damage and that I must guard against unpredictable actions by any dog I foster.

I promise to take adequate care of the pets that I foster while they are in my possession providing food, water, and shelter as needed. Further, I will comply with all applicable laws and ordinances pertaining to the pets in my care. I promise to notify FMN if any fostered pet is in need of veterinary care such as for medical issues or injuries that may arise, or for routine testing and refill treatment for existing conditions or illness so that this care can be provided in a timely manner.

At no time will I allow a foster pet in my care to be a nuisance to people or other animals. To that end I will keep it safely confined in a fenced yard or kennel run or inside a house or garage when it is not with me or a member of my family. At no time will the pet be placed outside on a tie-out unless under the direct supervision of a responsible adult. When off of my property, the pet will be kept on a leash unless it is in a fenced area approved for free running pets. I agree that the pet will not ride in the back of a pickup truck.

I understand that FMN has no prior knowledge of the health or temperament of its foster pets. I understand that FMN makes no warranties or statements regarding the foster pet's health or temperament. I agree and understand that FMN does not warrant a dog regarding medical status, behavior, personality, or disposition.

I further agree that FMN has no liability or responsibility of any nature regarding any physical or mental defects of the pet, any injuries or damage to any person or property caused by the pet, or any illness caused by the pet. I agree to indemnify and hold harmless FMN, its members or representatives, and the Fairbanks North Star Borough Animal Shelter against any and all claims for any injury, property damage, illness, or any other harm of any type or degree in the event that the pet is responsible, directly or indirectly for such injury, property damage, illness, or harm.

I understand that fostering a pet for FMN is not the same as adopting a pet from FMN and that this pet must be made available to prospective owners referred by FMN for viewing and possible permanent adoption. I understand that if I wish to adopt a pet which I foster, I must fill out an adoption application and pay the expected fee for the pet just as any other family.

I understand that I may not give, lease, sell, or otherwise transfer custody of the pet to anyone while it is under my care. I agree that FMN retains ownership of the pet while it is in my care, and any decisions regarding the pet or its placement will be made by FMN in its sole and complete discretion.

I understand that the foster animal(s) must remain in my care and control at all times with the obvious exceptions of pet adoptions, vet visits, and grooming and that if the foster dog needs temporary care for any reason, the foster agrees to contact the rescue to make arrangements with an approved representative

I agree to immediately surrender custody of the pet to FMN representative upon demand. I acknowledge that I have no legal ownership and will not obtain any legal rights to the foster pet in my care.
