To request an AMI/USA Continuing Professional Development Certificate, please complete the form below.

Course attendees will have two weeks after the conference concludes to apply for their Professional Development certificates. There will be a fee associated with the approval of applications submitted after this time.

All certificates will be sent via email within one week after the submission is received.

For CPD certficates from Sarasota University, applications can be submitted here.
