Tuition Assistance Form
If you are an active parishioner at CCSB and have children enrolled in Catholic school (grades PK-12), you are eligible to receive tuition assistance from the parish. These funds are required to be distributed by the Archdiocese; family income is not a consideration for eligibility. Please complete the form below by October 31; funds will be distributed directly to the school before the end of the calendar year. Please complete one form per child. Thank you!
