LET’S GO PLAY CHILDREN’S MUSEUM – PARTICIPATION, ASSUMPTION OF RISK & LIABILITY WAIVER

I understand that participation at Let’s Go Play Children’s Museum involves active, physical and imaginative play.

Activities and equipment may include, but are not limited to, climbing structures, a climbing wall approximately six feet in height, slides, a ball pit, interactive exhibits, imaginative-play structures, sensory activities and other play equipment.

I understand that these activities involve inherent and other risks, including falls, slips, trips, collisions with other participants or objects, loss of balance, contact with play equipment, and other injuries that can occur during active play.

I voluntarily allow the child(ren) listed above to participate with knowledge of these risks.

I understand that Let’s Go Play Children’s Museum is a family participation facility and does not provide childcare or individual supervision of children.

I agree that I, or another responsible adult accompanying my child, will actively supervise the child(ren) in my care while they are in the facility.

I agree to follow posted rules, staff instructions, age or equipment restrictions, and safety requirements.

Running, unsafe play, misuse of equipment, and behavior that may endanger the participant or others are prohibited.

Let’s Go Play Children’s Museum may restrict or discontinue a participant’s use of an exhibit or activity when reasonably necessary for safety.

I understand that grippy socks are required for participating children while using designated play areas at Let’s Go Play Children’s Museum.

I understand that floors and play surfaces may present slipping or tripping hazards and agree to ensure that the child(ren) in my care comply with the museum’s footwear requirements.

In consideration for being permitted to participate in activities at Let’s Go Play Children’s Museum, I voluntarily assume the inherent and reasonably foreseeable risks associated with the activities and equipment described in this agreement.

To the extent permitted by Wisconsin law, I agree to release and hold harmless Let’s Go Play Children’s Museum, its organization, directors, officers, employees, volunteers and agents from claims arising from the ordinary risks inherent in participation and from ordinary negligence associated with the activities and conditions specifically described in this agreement.

This agreement is not intended to release claims that cannot lawfully be waived under Wisconsin law.

I understand that by agreeing to this provision I may be giving up certain legal rights for myself and/or the minor child(ren) identified above.

This section is optional and does not affect admission or participation. Please select one: YES – I give Let’s Go Play Children’s Museum permission to photograph or record the child(ren) listed above and to use those images or recordings for museum-related promotional, educational, social media, website and community-outreach purposes. NO – I do not give permission for the child(ren) listed above to be intentionally photographed or recorded for these purposes.

By signing below, I acknowledge that: • I have read this entire agreement. • I understand its terms. • I have had the opportunity to ask questions before signing. • I understand that this agreement contains an assumption of risk and release of certain legal rights. • The information I provided is accurate. • I am the parent/legal guardian of the minor child(ren) listed above or otherwise have legal authority to enter into this agreement on their behalf. • I am signing this agreement voluntarily.
