Please note that this is an application form, and submission does not guarantee your booking. All applications will be reviewed by our team, and we will contact you to confirm your participation once your application has been approved.

Unless you have purchased additional places, your package includes the following number of delegate spaces:

- Platinum & Gold: 4 delegates
- Silver: 3 delegates
- Bronze & Exhibitor: 2 delegates

Once your booking is confirmed, we will contact the named individuals directly with registration details. This will allow us to collect dietary requirements and ensure they receive all relevant updates.

All applications are carefully reviewed to ensure that our event supporters align with our values. Before submitting your application, please read and agree to our Working with ADCH Code of Conduct and any terms that apply to your selected package.

It is the responsibility of the primary contact to notify ADCH directly if any details about your application change prior to the event.

The information you provide will be stored electronically by ADCH and used in connection with your application.

This may include sharing your information with third parties, as necessary, to assess or facilitate your application.
