I agree to abide by the Governing Documents of Ontario Soccer and the District Association; and abide by the decisions made by the District Association's Board of Directors. 

I agree to allow EMSA to provide by name and contact information to any club within EMSA District for the use of evaluating Grassroots players via the use of a Fast Track Form. At that point, it is between myself and the player/parent/team/club to make any further arrangements.

My assessment will be based on the best interest of the player and I agree to maintain and uphold my professional integrity throughout this process. 

As a TD, I will submit to to EMSA the Fast Track Forms via Ontario Soccers' online JotForm required for fast tracking within 7 days of the evaluation.
