I am aware of the dangers of attending Restore Church's Monday evening dance class for Special Needs individuals. I promise not to hold Restore Church, its officers, agents, or employees liable or pursue legal action against them. This waiver absolves Restore Church of any duty for injuries sustained on the premises before to, during, or after the activity.
