This agreement is between Sorella Behavioral Solutions LLC, an Arizona limited liability company ("Sorella"), and the client named below. Questions and notices may be sent to Sorella at sorellaabsaba@gmail.com

Purpose and contractual relationship. By signing, Sorella and Applicant establish a contractual relationship for the limited purpose of Applicant participating in an RBT Initial Competency Assessment with a consenting Sorella client. This agreement does not make Applicant a Sorella employee or establish ongoing supervision after the assessment.

What Applicant will do. Applicant may join a live virtual social skills session to demonstrate assigned skills with a client under the direction of the responsible BCBA assessor. Applicant will follow the assessor's instructions, act professionally, and stop or change an activity when directed. Applicant may participate only when the client or guardian has consented.

Privacy and boundaries. Applicant will keep client information confidential and use it only for the authorized assessment. Applicant will join from a private location and will not record, photograph, copy, retain, post, or share client information or session content. Applicant will not contact or work with the client or family outside activities Sorella authorizes.

End of agreement. This limited arrangement ends when the assessment is completed or Sorella ends Applicant's participation. Sorella may end participation for safety, privacy, or professional conduct concerns. Applicant's duty to protect client information continues afterward. The responsible assessor determines whether skills were demonstrated; RBT certification is determined by the BACB.
