By ticking this declaration, I confirm my acceptance and acknowledgement of these documents electronically in place of signing the individual biodata and employment history documents.
I authorise R.C Employment Pte. Ltd. to carry out the necessary MOM Work Permit transactions on my behalf, where applicable to my application or employment arrangement, including application, issuance, renewal, extension, cancellation and/or transfer of my MDW’s Work Permit, and any related transactions reasonably required to process or complete the application.
Where a handover or overlapping period between my current and incoming MDW is required or approved, I understand that I may be liable for the applicable levy for both MDWs during the overlapping period.
I declare that all information and documents provided by me are true, complete and accurate. I acknowledge that R.C Employment Pte. Ltd. may rely on the information, documents, instructions, confirmations and authorisations provided by me, including those given electronically through messaging platforms, email, Singpass/MOM systems or other digital communications, as my instructions and authorisation to proceed.
I declare that I do not have any child/children below 7 years old who has/have not received the necessary diphtheria and measles vaccinations under the Singapore National Childhood Immunisation Schedule. If any child below 7 years old has not received the required vaccinations, I acknowledge and agree that I am responsible for taking the MDW for the required immunisation(s) and ensuring that the necessary immunisation(s) are completed.
