Social Club Application

Grassroots K9 Georgina
Social Club is a structured day training program. Dogs are placed in a fixed group of up to eight on an assigned day each week, because group stability is the program. Every application is reviewed individually, and accepted dogs complete a paid assessment before an offer of a place.

Complete every question as fully and honestly as you can. Incomplete answers may delay your application, and undisclosed behaviour history is grounds for removal from the program. Return the completed form with your dog's vaccination records attached.

Applications close October 20th. Applications are reviewed in the order received.
