I certify that I have disclosed all known medical conditions, allergies, medications, skin problems, seizures, heart or respiratory conditions, mobility limitations, recent illness/injury, and other information that may affect grooming. I have also disclosed any known history of biting, snapping, aggression, fear, escape behavior, or difficulty with grooming, handling, dryers, clippers, nail trimming, or restraint. I also certify that all pets that will be serviced have all required vaccinations.

I authorize the groomer to provide agreed grooming services to my pets. I understand that grooming involves handling, bathing, drying, brushing, clipping, trimming, nail care and/or other services and that even with reasonable care, unexpected reactions or injuries can occur. These may include minor cuts, clipper irritation, nail bleeding, stress, skin irritation, or discovery/aggravation of a pre-existing condition. I agree to promptly inform the groomer of any concern that could affect safe grooming.

I understand that severe matting can conceal skin problems and increase the risk of irritation, bruising, cuts, hematomas, or other complications. When the groomer believes extensive de-matting would cause unreasonable discomfort or risk, I authorize the groomer to choose a humane alternative, including clipping the coat shorter, after discussing the plan with me when reasonably possible.

I agree to provide a reasonably safe, sanitary, well-lit and accessible area suitable for grooming, with any agreed access to water and electricity. I will secure other animals and keep children, guests, or others from interfering with the groom unless the groomer agrees otherwise. I will disclose known hazards that could affect the groomer's safety or ability to work. The groomer may refuse to enter, stop services, or leave the premises at any time if the groomer reasonably believes the environment, animal, or circumstances are unsafe, threatening, unsanitary, inappropriate, or unsuitable for grooming. No future appointment or ongoing client relationship is guaranteed.

If my dog appears to need urgent veterinary attention during the appointment and I cannot be reached promptly, I authorize the groomer to contact my veterinarian, an emergency veterinary facility, or emergency services as reasonably necessary. I understand that I remain responsible for veterinary charges except to the extent the groomer is legally responsible or applicable insurance provides coverage. The groomer will make reasonable efforts to contact me as soon as practicable.

I will identify any special access instructions and will provide a safe location for the groomer's equipment. I understand that the groomer will exercise reasonable care while working in my home. I agree to report any alleged property damage or incident promptly so it can be documented and, when appropriate, submitted to insurance.

Payment is due upon completion of the services and is what is listed on the business' website unless otherwise agreed upon with the groomer. I will provide notice of cancellation/rescheduling at least 24hrs before my scheduled appointment time unless it is an emergency. (At this time there is no cancellation fee.)

The groomer reserves the right to decline, modify, discontinue, or refuse current or future services when reasonably necessary for the safety or welfare of the dog, groomer, client, or others; because the home environment is unsuitable; because required information was not disclosed; or because the client/service arrangement is not a suitable fit for the business. When practical, the client will be informed that the service has been stopped or cannot be continued.

I understand that this agreement does not waive rights or responsibilities that cannot legally be waived. To the extent permitted by applicable law, I accept the ordinary and inherent risks of grooming that are not caused by the groomer's negligence, gross negligence, willful misconduct, or other liability that cannot legally be excluded. Nothing in this agreement is intended to release the groomer from liability where such a release would be unenforceable under applicable law.

I certify that the information I provided is accurate to the best of my knowledge. I have read and understand this agreement, had the opportunity to ask questions, and voluntarily agree to its terms. By typing my name below, I agree that it constitutes my electronic signature.
