Parent/Guardian Consent & Release
I give permission for the minor named above to volunteer with ArtHaus Foundation, Inc. during the 2026 ArtHaus Family Arts Festival and related activities.
I understand that participation in an outdoor community event may involve risks, including but not limited to physical activity, weather conditions, equipment, crowds, vehicles, and other event-related hazards. I knowingly allow my child to participate and, to the fullest extent permitted by law, release and hold harmless ArtHaus Foundation, Inc., its directors, officers, employees, volunteers, and agents from claims arising from my child’s participation, except as otherwise provided by law.
Emergency Medical Care: In the event of an emergency and if I cannot be reached, I authorize ArtHaus representatives to obtain reasonable emergency medical assistance for my child. I understand that I am responsible for any resulting medical expenses.
Photography/Video: I give ArtHaus Foundation permission to photograph or record my child during the event and to use those images or recordings for ArtHaus educational, promotional, social media, website, and nonprofit purposes without compensation.
By signing below, I acknowledge that I have read and understand this form and voluntarily give permission for my child to participate.
