Choose the adult or parent/legal guardian agreement below. Adults sign only the adult agreement for themselves. A parent or legal guardian signs the minor agreement for a child.
MAOZ Academy Adult Participation Waiver
Assumption of risk and release of liability | Adults age 18 and older | Version 2026-10-04
READ BEFORE SIGNING. THIS AGREEMENT RELEASES CLAIMS FOR ORDINARY NEGLIGENCE AND AFFECTS YOUR LEGAL RIGHTS.
Legal operator: MA’OZ Academy doing business as MA’OZ Academy (“Academy”). Facility: 638 N. Main St., Suite B, Taylor, AZ 85939.
The “Released Parties” are MA’OZ Academy and Momentum Gym; their respective owners, members, managers, employees, instructors, contracted instructors and volunteers; and the facility owner and lessor, each only in connection with the covered Academy activities.
1 Activities and duration
In exchange for permission to participate, I agree that this agreement covers my Academy-organized jiu-jitsu, grappling, takedowns, wrestling, self-defense instruction, physical conditioning, drills, sparring, open mat, trials, private lessons, demonstrations, seminars and Academy-organized events, and related use of its premises and equipment. It applies from signing through the next 12 months, including repeated visits and approved off-site Academy activities. Independent events organized by others are outside this agreement.
2 Specific risks
These are contact activities. Risks include falls and throws; impact with people, floors, walls or equipment; twisting, compression and joint locks; neck pressure and choking techniques; restricted breathing or blood flow; inability to tap or a delayed response to a tap; mismatched size, skill or strength; unexpected partner movements; excessive force; and accidental strikes. Injuries may include bruises, cuts, burns, sprains, dislocations, broken bones, dental and eye injuries, concussion and other brain injury, neck or spinal injury, nerve injury, loss of consciousness, paralysis, permanent disability and death. Conditioning may cause exhaustion, dehydration, heat illness or cardiac events. Shared surfaces and close contact may transmit skin infections and other communicable illnesses. Slips, trips, mat gaps, equipment defects, crowding and personal-property loss are also possible. A risk may exist even when instructions and safety rules are followed.
3 Voluntary assumption of risk
I understand these risks, including the possibility of severe injury or death, and voluntarily choose to participate. To the fullest extent Arizona law permits, I assume risks arising from the covered activities, including risks caused by a Released Party’s ordinary negligence. I may decline a drill, partner or technique, stop participating, and ask questions. No one has promised that training, a technique, protective equipment or supervision will prevent injury.
4 Express release of ordinary negligence
TO THE FULLEST EXTENT PERMITTED BY ARIZONA LAW, I RELEASE THE RELEASED PARTIES FROM MY CLAIMS FOR PERSONAL INJURY, ILLNESS, DEATH AND PROPERTY DAMAGE ARISING FROM THE COVERED ACTIVITIES, INCLUDING CLAIMS CAUSED IN WHOLE OR IN PART BY THEIR OWN ORDINARY NEGLIGENCE. This includes negligent instruction, supervision, partner selection, assessment of readiness, enforcement of rules, inspection, maintenance, cleaning, warnings, equipment selection and emergency response. This release applies to claims I have authority to release; it does not purport to release independent claims belonging to other people.
This agreement does not release gross negligence, reckless or intentional misconduct, abuse, or any liability or legal right that cannot lawfully be released. It does not restrict reporting misconduct to authorities or cooperating with an investigation.
5 Participant duties and health
I will follow Academy rules and instructor directions, release immediately when a partner taps or verbally submits, use controlled force, and avoid techniques or activities for which I have not received permission. I will report injuries, unsafe conditions and relevant limitations before continuing, refrain from training while impaired or with a potentially contagious condition, and stop if I experience concerning symptoms. I understand that instructors do not provide medical clearance. I will obtain medical advice when needed for my condition and will not knowingly conceal a limitation affecting safe participation.
6 Emergency assistance and expenses
I authorize Academy personnel to request emergency assistance and provide first aid within their training if necessary. Qualified medical personnel determine treatment under applicable law. I remain responsible for expenses for which I am legally liable. The Academy makes no promise to provide personal medical insurance or pay medical expenses. This authorization does not expand the negligence release beyond Section 4.
7 Responsibility for harm to others
To the extent permitted by law, I will reimburse a Released Party for third-party liability and reasonable defense costs attributable to my intentional misconduct or negligent violation of safety rules. This obligation is limited to my share of fault. It does not require me to reimburse liability attributable to a Released Party’s own negligence or other misconduct, or penalize a lawful claim that this agreement does not release.
8 Governing law and complete agreement
Arizona law governs. Any action will be brought in a court with jurisdiction in the Arizona county where the facility is located, unless applicable law requires otherwise. If a provision is unenforceable, the remaining provisions remain effective to the extent permitted by law. This document contains the entire agreement concerning its subject; changes require written agreement. No oral statement changes it. This agreement does not waive a nonwaivable jury right or require arbitration.
9 Signature and acknowledgment
I am at least 18, have read the entire adult agreement, understand the specific risks and ordinary-negligence release, have had an opportunity to ask questions, and sign voluntarily. If signing electronically, I agree to conduct this transaction electronically and intend my electronic signature to be my signature. I may obtain a copy.
PARENT / LEGAL GUARDIAN AGREEMENT FOR A MINOR
MAOZ Academy Minor Participation Agreement
Parent or legal guardian consent and personal release | Version 2026-10-04
Legal operator: MA’OZ Academy doing business as MA’OZ Academy (“Academy”). Facility: 638 N. Main St., Suite B, Taylor, AZ 85939.
The “Released Parties” are MA’OZ Academy and Momentum Gym; their respective owners, members, managers, employees, instructors, contracted instructors and volunteers; and the facility owner and lessor, each only in connection with the covered Academy activities.
1 Permission and acknowledgment of risks
I am this child’s parent or legal guardian with authority to authorize participation. I consent to Academy-organized jiu-jitsu, grappling, takedowns, wrestling, self-defense, conditioning, drills, supervised sparring, open mat, trials, private lessons, demonstrations, seminars and events, including approved off-site activities, for 12 months after signing. I acknowledge risks of falls, collisions, accidental strikes, joint locks, neck pressure and chokes, delayed release after submission, mismatched partners, overexertion, unsafe surfaces or equipment, and communicable infection. These can cause broken bones, dislocations, concussion, brain or spinal injury, unconsciousness, paralysis, disability or death even with instruction and supervision. I have discussed age-appropriate safety rules and stopping or tapping with my child. I may decline a partner or activity for my child.
2 Parent or guardian personal release
TO THE FULLEST EXTENT ARIZONA LAW PERMITS, I RELEASE ONLY MY OWN CLAIMS ARISING FROM THE CHILD’S COVERED PARTICIPATION, INCLUDING CLAIMS CAUSED BY THE RELEASED PARTIES’ ORDINARY NEGLIGENCE in instruction, supervision, partner selection, rule enforcement, premises or equipment maintenance, warnings, cleaning or emergency response. This includes my own legally releasable claims for expenses or loss resulting from the child’s injury. It does not release gross negligence, reckless or intentional misconduct, abuse, or nonwaivable liability.
I acknowledge the child’s risks and consent to participation. This agreement does not represent that my signature extinguishes the child’s independent legal claims. I do not promise to indemnify the Academy for the child asserting such claims. My personal release is separate from my consent and remains subject to applicable law.
3 Safety and emergency care
I will communicate relevant limitations, keep emergency contacts current, and follow drop-off and pickup rules. My child must immediately release when a partner taps or verbally submits, follow instructions, and report injury or discomfort. I authorize staff to call emergency services and provide first aid within their training when needed. Medical professionals determine treatment under applicable law. I am responsible for expenses for which I am legally liable. No personal medical insurance is promised by the Academy.
4 Agreement and signatures
Arizona law governs. An invalid provision does not invalidate the remaining provisions where lawful. I have read this entire page, had an opportunity to ask questions, and sign voluntarily. If signing electronically, I consent to electronic transactions and intend my electronic signature as my signature. I may obtain a copy. This consent does not restrict reporting misconduct or cooperating with authorities.
