    By checking the box below, I authorize the Union County Fiber Accountability Coalition and its organizers to review, organize, summarize, and share the information and supporting materials I submit, including photographs and documents, for the purposes of investigating fiber construction incidents, seeking repairs or resolution, and advocating for safety and accountability.

    Relevant information may be shared with government officials and agencies, attorneys, journalists and investigative news teams, utility companies, fiber companies, and contractors involved in investigating or resolving these concerns.
