Pre-Qualification Form

The business identified above (“Business”) and each undersigned owner or officer acknowledge and agree that: (1) all information and documents provided to CashFlow2Go LLC (“CashFlow2Go”) are true, accurate, and complete, and they will promptly notify CashFlow2Go of any material changes while the financing request is pending; (2) they authorize CashFlow2Go to review and verify the information provided and to share the application and supporting documents with third-party financing providers identified to them and authorized to receive their information, for the purpose of evaluating the Business’s request for commercial financing; (3) those providers may review and verify the information and request additional documentation necessary to evaluate the request, subject to applicable law and any additional authorizations required; (4) any personal credit report or investigative consumer report will be obtained only pursuant to a legally permissible purpose and any required individual authorization, and this paragraph does not authorize unrestricted access to or redistribution of those reports; (5) CashFlow2Go provides financing referral and application-support services and does not itself approve or fund financing; financing providers independently determine eligibility, approval, and terms; and (6) each person signing represents that they are authorized to submit this application and authorize disclosure of the Business’s information on its behalf. No signer purports to authorize access to another individual’s personal credit report.
