Notice to the minor child’s natural guardian:
Read this form completely and carefully. You are agreeing to let your minor child engage in a potentially dangerous activity. You are agreeing that even if First Baptist Church of Orange Park, its ministries, its employees, agents, and volunteers use reasonable care in providing this activity, there is a chance your child may be seriously injured or killed by participating in this activity because there are certain potential dangers inherent in the activity which cannot be avoided or eliminated. By signing this form, you are giving up your child's right and your right to recover from First Baptist Church of Orange Park, its ministries, or any of its employees, agents, or volunteers in a lawsuit for any personal injury including death, to your child or any property damage that results from the risks that are a natural part of the activity. You have the right to refuse to sign this form, and First Baptist Church of Orange Park has the right to refuse to let your child participate if you do not sign this form.
Travel/Vehicle Permission: I give permission for the minor participant designated on this form to ride in vehicles approved by the FBCOP leadership for ministry events in accordance with the church’s safety protocols regarding minors. If participant is injured while riding in a church-approved vehicle for an event or activity, he/she will be primarily covered for bodily injury under our personal automobile insurance policy.
Photography/Video Acknowledgment: Throughout events the church hosts or attends there may be photographs and video taken for promotional purposes. I acknowledge that and grant permission for such media to be used in promotional materials, in print and online.
Release and Indemnity: I acknowledge and agree that I or my agents hold harmless, release forever, and agree not to sue First Baptist Church of Orange Park, its leadership, agents, venues, locations, volunteers, or sponsors from any and all claims or demands related to personal injury, sickness, and even death, as well as any property damage or related expenses incurred by my participation or my minor child during church sponsored events or activities. In the event of a medical need, I understand that the authorized agent of First Baptist Church is responsible for care and decisions related to medical needs including, but not limited to medical consent, care, transportation, and communication with the family. Any and all medical expenses that could be incurred if medical care is needed are my sole responsibility and I release liability and understand that I or my minor participate in any and all activities at will.
Understanding: I acknowledge that I have read and understand this waiver and release and all its terms, and my signature below represents that understanding and I freely relinquish legal rights, I have had the opportunity to obtain any and all counsel if needed and that by my signature, I understand and accept this agreement in full. Furthermore, it is understood that a copy of this form is treated as authentic and binding as the original.
