I am requesting a free trial visit at High Definition Fitness (the "FACILITY"). I understand that this is a trial visit only, not a membership, and involves no payment or commitment.

Physical Condition. I am in good physical condition and have no medical reason, impairment or disability that might prevent me from exercising. If I have health concerns, I will speak with a physician before my visit.

Assumption of Risk. I understand that using the FACILITY and its equipment carries a significant risk of injury, including the potential for permanent paralysis and death. I knowingly and freely assume all such risks, both known and unknown.

Release. I release, indemnify and hold harmless TrueSport Fitness Ltd., its shareholders, the owners and lessors of the premises, and their respective officers, affiliates, agents and employees from any and all injury, disability, death, loss or damage to person or property arising out of or in connection with my trial visit or my use of the FACILITY or its equipment, including claims based on negligence. This release is intended to be as broad as permitted by law; if any part is held invalid, the rest remains in effect. It applies to my heirs, assigns, personal representatives and next of kin.

Rules. During my visit I will follow all FACILITY rules, the Terms & Conditions, and the directions of staff. I will not bring guests, take photos or videos, or use equipment I am unsure how to use without instruction.

Governing Law. This waiver is governed by the laws of the Province of Alberta.

I certify that I am at least 18 years of age, have read this waiver and the Terms & Conditions, understand that I am giving up substantial rights by signing it, and sign it freely and voluntarily.
