PACKAGE TERMS & CONDITIONS

1. Package Expiry: All purchased InBody scans are valid for 24 months from the date of purchase. Any unused scans remaining after 24 months are forfeited and are not eligible for refund, transfer or extension unless otherwise approved by High Definition Fitness.

2. Non-Refundable: InBody scan packages are non-refundable once purchased.

3. Non-Transferable: Packages are for use by the purchasing customer and may not be transferred or shared with another person unless approved in writing by High Definition Fitness.

4. Use of Scans: Each scan in the package is one InBody assessment. Additional services or consultations, if applicable, may be subject to separate fees.

CUSTOMER ACKNOWLEDGEMENT

By signing below, I acknowledge that I have selected and purchased the InBody scan package indicated above. I understand and agree that my purchased scans expire 24 months from the date of purchase and that any unused scans after that date will be forfeited. I have had the opportunity to ask questions about the package and its terms and agree to the conditions above.
