REGISTRATION WILL CLOSE ON SUNDAY, OCTOBER 11TH

PLAYERS WILL BE PLACED ON TEAMS BASED ON EXPERIENCE AND ABILITY LEVELS.  CERTAIN TEAMS MAY BE A MIXTURE OF BOTH 7TH AND 8TH GRADE PLAYERS.

UNIFORMS

Players will wear reversibles from school or past in house league as long as they are black on one side and white on the other and have the following screen on the front and a number on the back of each side. See picture below.

If not, you will need to purchase one at check out so all players match. If you own one, please be sure to enter the number of the reversible below when asked.

All Players will receive a program T-shirt

CLUB 1 WAIVER

We have a waiver below that is for our program that you will compete with this registration. Club 1 will also be sending me a link for a waiver that you must complete for them. I will send it to you as soon as I receive it. It must be completed before the first game. Thank you!

PRACTICE:

Teams will practice at least one a week.  Practice schedules will be released soon. 

GAME DATES ARE AS FOLLOWS:

November 8, 15, 22, Dec. 6:  two games for the first four Sundays

Dec. 13– single elimination tournament

TRANSPORTATION TO AND FROM GAMES:

Parents are responsible for providing transportation to and from games to area schools.  Practices will most likely be at Chesterton High School but may occur at other Duneland School locations.

Permission For Emergency Medical Treatment

As the legal parent or guardian, in the event of illness or accident, I give consent for the Chesterton Girls Basketball LLC and/or the members of the Chesterton High School Basketball Coaching Staff to secure any and all necessary emergency medical care for my child.  The undersigned gives permission to Chesterton Girls Basketball LLC, its owners and operators and members of the Chesterton High School Basketball Coaching staff to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I request that my child be transported to an area emergency facility in the event of an emergency.  I hereby declare we have listed any physical/mental problems, restrictions, or conditions and/or declare the participant to be in good enough physical and mental health to participate in basketball practice and games.  

Release of Liability

As the legal parent or guardian, I release and hold harmless Chesterton Girls Basketball LLC, and/or the its owners and operators, members of the Chesterton High School Basketball Coaching Staff and Players, and the Duneland School Corporation from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Chesterton Girls Basketball LLC, its owners and operators or in route to or from any of said premises.  I also attest that my child/children are in sound physical health and able to safely compete in rigorous physical activity.

Social Media Release:

As the legal parent or guardian, I grant permission for the Chesterton Girls Basketball LLC and/or Chesterton High School Girls Basketball Program to utilize my player's photo or video footage to promote the program using various social media outlets and video promotions.

Spectator Code Of Conduct Agreement:

It is important that every spectator be an exemplary role model and maintain the highest standard of conduct at all times. Spectators represent their community, school and team. Many people observe their conduct closely. The spectator must recognize that the purpose of school based activities is to promote physical, mental, moral, social and emotional well-being of participants. The following guidelines will be upheld with all spectators at games:

• Chesterton Basketball Fans WILL:
  • Cheer with respect and in the spirit of good sportsmanship. Treat all participants with respect.  Support the efforts of players and coaches regardless of the result. Comply with any directions given by game management.
• Chesterton Basketball Fans WILL NOT: Ridicule, criticize, or show any signs of disrespect to officials, players, coaches, or other spectators. Use vulgar, racial, culturally insensitive or abusive language or threatening gestures. Engage in any activity that embarrasses others or prohibits them from enjoying the experience of the game. Attend any game under the influence of alcohol or drugs. I, THE PARENT/GUARDIAN UNDERSTAND THE ABOVE EXPECTATIONS AND UNDERSTAND THAT I WILL BE ASKED TO LEAVE THE SESSION/LEAGUE/GAME AND WILL NOT BE PERMITTED TO ATTEND FUTURE LEAGUE PRACTICES OR GAMES THAT SEASON.
