I acknowledge by signing this document that NINJEW TATTOO has given me full opportunity to ask all questions I might have regarding obtaining a tattoo. All of my questions have been answered to my complete satisfaction. My signature also acknowledges that I have been specifically advised about the facts and matters set forth below and my agreement to the following:
I do not have hemophilia, nor do I have diabetes, epilepsy, hepatitis, HIV, AIDS, or any other communicable disease. I am not under the influence of alcohol and/or any drugs. I acknowledge that it is not reasonably possible for NINJEW TATTOO to determine whether I might have an allergic reaction to the pigments or process used in my tattoo, and I agree to accept that such a reaction is possible. I acknowledge that infection is always possible as the result of obtaining a tattoo, particularly in the event that I do not take proper care of the tattoo, and I agree to follow all instructions concerning the healing process. I agree any touch-up work required due to my own negligence will be done at my own expense. I realize that variation of colour and design may exist between the tattoo I have selected and how it appears on my skin when compared to someone else. I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove the tattoo. To my knowledge, I do not have any physical, mental, or medical impairment or disability that may affect my wellbeing as a direct or indirect result of my decision to have any tattoo related work done at this time. I acknowledge that I have truthfully represented to NINJEW TATTOO that I am over the age of 18 years, or have the full consent of my parent/guardian and over the age of 16. I acknowledge that obtaining my tattoo is my choice alone, and I consent to the application of the tattoo and to any action or conduct reasonably necessary to preform the tattoo procedure. I agree to release and forever discharge and hold harmless NINJEW TATTOO from any and all claims, damages, and legal actions arising from or connected in any way with my tattoo or the procedures and conduct used to apply my tattoo.
