Thank you for sponsoring the 10th Annual Words & Wisdom on April 22, 2027!
Please note, this form is only for sponsors who will be paying via a donor advised fund (DAF), IRA qualified charitable distribution (QCD), family foundation, or have a pre-existing Corporate Partnership Agreement with CHF.

Sponsors paying with personal or corporate funds via credit card or invoice request (check or ACH transfer) should complete this form instead.

Children's Hospital Foundation is a 501(c)(3) non-profit organization. The portion of your gift that is not tax-deductible is $88 per seat. Please contact us at wisdom@chfrichmond.org with any questions.

- Digital ads will be displayed in 16:9 aspect ratio (widescreen) at 1920p x 1080p. Digital artwork is preferred as a PowerPoint or PDF file. We can also accept high resolution JPEG and PNG files. A blank PowerPoint template can be downloaded here.

- Will payment be made using a Donor Advised Fund (DAF), IRA via qualified charitable distribution (QCD), a family foundation, or through a pre-existing Corporate Partnership Agreement with CHF?
- I understand that since payment is coming through a Donor Advised Fund (DAF) I am not entitled to benefits with monetary value, including event seats. If I wish to attend the event, I will purchase tickets separately. Further, I understand that CHF honors all requirements specified in the documentation accompanying DAF payments. This may include prohibitions against applying funds to event support. In applying for the gift, I will follow the DAF instructions to have the accompanying documentation state the purpose as support for the event and event designation.
- I understand that I, as the sponsor, am responsible for complying with IRS regulations specific to receipt of benefits for IRA payments via a qualified charitable distribution (QCD). I understand I am not entitled to benefits with monetary value, including event seats. If I wish to attend the event, I will purchase tickets separately.
