1. Authorization to repair. I certify that I am the owner of the vehicle described above, or the owner's authorized representative, and I authorize Lone Star Dent Repair ("LSDR") and its technicians to perform paintless dent repair and all related repair operations on the vehicle, to operate it as necessary for inspection, testing, and delivery, and to photograph it before, during, and after the repair for claim documentation.
2. Limited power of attorney. I give LSDR limited permission to handle this claim on my behalf: to speak with my adjuster, appraiser, and carrier, to request and negotiate estimates and supplements, to obtain copies of my claim documents and photos, and to endorse an insurance check made payable to both of us, limited to the amount LSDR has earned on my repair. This covers this claim only. It does not allow LSDR to settle or close my claim, and it ends when my bill is paid in full.
3. Direction of pay. I direct my insurance company to send payment for these repairs straight to LONE STAR DENT REPAIR, and I assign those proceeds to LSDR up to the amount of my final bill. If a check comes to me instead, I will sign it over within three business days. If my insurance pays less than the bill, I still owe the difference, and my deductible is my responsibility.
4. Warranty and limitation of liability. LSDR warrants its workmanship on the panels it repairs for as long as I own the vehicle, and the remedy is re-repair of a covered panel at LSDR's facility. This is the only warranty provided; all other warranties, express or implied, are disclaimed to the extent permitted by law. LSDR's liability is limited to the amount I paid for the repair.
