During regularly scheduled evenings and special events, our youth group often uses photographs and videos of our students for a variety of projects and media. Because we are sensitive to your family's safety and privacy, at no time will the names of our students accompany their photos or videos. Below is a release that allows you to indicate your preferences
Photo/Media Usage Consent

I give permission for HSEC to use photos and other media formats of my child(ren) on the church website and other forms of communication and publications.
I do NOT give permission for my child(ren)'s photos to be used.
Program Participation Consent
I hereby give permission for my child to attend and participate in Children & Youth Ministry events sponsored by Holy Spirit Episcopal Church (HSEC).
In consideration of my child being allowed to participate in HSEC children & youth group activities, I hereby assume all risks and release HSEC, its employees, and volunteers from all liability whatsoever for any injuries or accidents in connection with my child's participation. I intend this release to be binding for myself and my family and all legal successors in interest.
In the event that my child is injured, I hereby give permission to the physician or medical personnel selected by HSEC staff or volunteers to hospitalize, secure proper treatment or medication for, and to take whatever medical actions are necessary to treat my child, and I authorize the physician or medical personnel to provide treatment deemed necessary to them.
