2) Scope of Services
Consultant will perform mold testing at the Service Address. This agreement includes the collection and laboratory analysis of three (3) samples: two (2) air quality samples and one (1) surface swab sample. The testing is limited to the samples collected and the conditions observed at the time of service.
3) Important Limitations
This assessment is non-invasive and represents a snapshot in time. Mold spores and conditions may change based on weather, HVAC operation, cleaning, or other factors. No guarantee is made that all mold sources will be identified or that conditions will not change after sampling. Consultant does not provide medical advice or determine individual health risks.
4) Client Responsibilities
Client will provide safe access, disclose prior water events or remediation, and ensure HVAC is operating under normal conditions prior to testing.
5) Fees & Payment
The services and sample fees covered by this Agreement are listed below. Each sample is $125.00, for a total of $375.00 for three (3) samples. Any additional samples, rush analysis, or services not listed in this Agreement must be agreed to separately and may be billed as additional charges.

6) Liability & Indemnification
Consultant will perform services in accordance with generally accepted industry practices. However, Consultant's total liability for any claim, whether in contract, tort, or otherwise, shall not exceed the total fees paid under this Agreement. Consultant is not liable for indirect, incidental, or consequential damages, including but not limited to loss of use or profit. Client agrees to indemnify and hold harmless Consultant from any third-party claims arising from Client's use of the findings or failure to act upon recommendations.
7) Deliverables
Consultant will provide a written report summarizing observations, laboratory results, and recommendations. Implementation of recommendations and remediation is the Client's responsibility.
8) Authorization to Proceed
By signing below, Client authorizes Consultant to perform the services described and agrees to the terms of this Agreement.
