Friendsgiving with Bishop Rojas — Register 🍂

Photo & Video Release I grant the Roman Catholic Bishop of San Bernardino, a corporation sole, including the Diocese of San Bernardino, its Office of Vocations, parishes, and agents (together, the "Diocese"), the irrevocable right to photograph and record video and audio of me (and of any minor registered under this form) at Friendsgiving with Bishop Rojas on November 5, 2026, and to use, reproduce, edit, publish, and distribute those images and recordings, including name of parish but not home address or contact information, in any media now known or later developed, including websites, social media, print materials, newsletters, and promotional videos, for ministry, educational, and promotional purposes, without compensation. I waive any right to inspect or approve the finished material. I release the Diocese from any claims arising from such use, including claims for invasion of privacy, right of publicity, or defamation. The Diocese will not identify minors by full name in public materials without separate written permission. If I do not wish to be photographed, I will let a staff member know at check-in and will be given a colored wristband.

Assumption of Risk, Release of Liability & Medical Authorization 1. Voluntary participation. I am voluntarily participating (or permitting my minor child to participate) in Friendsgiving with Bishop Rojas, including Eucharistic Adoration, dinner, and related activities (the "Event"). 2. Assumption of risk. I understand that participation involves risks, including but not limited to slips, trips and falls, food allergies or foodborne illness, communicable illness, and injury or loss of personal property. I knowingly and voluntarily assume all such risks, both known and unknown. 3. Release and waiver. To the fullest extent permitted by law, I, for myself, my heirs, executors, and assigns, release, waive, and discharge the Roman Catholic Bishop of San Bernardino, a corporation sole, the Diocese of San Bernardino, Christ the Redeemer Parish, and their clergy, employees, volunteers, and agents (the "Released Parties") from any and all claims, liabilities, and causes of action for personal injury, illness, death, or property damage arising out of or related to the Event, including claims arising from the ordinary negligence of the Released Parties. This release does not apply to gross negligence or willful misconduct. 4. Indemnity. I agree to indemnify and hold harmless the Released Parties from claims brought by me or on my (or my minor's) behalf arising from my or my minor's participation, except to the extent caused by the Released Parties' gross negligence or willful misconduct. 5. Conduct. I agree to follow the instructions of event staff and the Diocese's Code of Conduct. The Diocese may ask any participant to leave for unsafe or inappropriate behavior. 6. Medical authorization. In the event of an emergency, I authorize event staff to obtain emergency medical treatment for me (or my minor) and understand I am responsible for any resulting costs. Staff will make reasonable efforts to contact the emergency contact listed. 7. Food. I am responsible for disclosing allergies and dietary restrictions above. The Diocese cannot guarantee any food is free of allergens. 8. Severability; governing law. If any part of this agreement is held invalid, the remainder stays in effect. This agreement is governed by the laws of the State of California. I HAVE READ THIS DOCUMENT, UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS, AND SIGN IT VOLUNTARILY.
