Release of Liability, Assumption of Risk, and Participant Agreement

In consideration of being permitted to participate in House’s Run, Walk, Give 2026 fundraiser (“the Event”), held on Saturday, December 12, 2026, starting at Crossroads Community Church, 505 Gahagan Rd, Summerville, SC, I agree to the following:

1. Voluntary Participation. I am participating in the Event voluntarily. I understand that running and walking involve physical exertion and certain risks, including but not limited to falls, collisions, uneven surfaces, traffic, weather conditions, dehydration, heat or cold-related illness, and other injuries, some of which may be serious.

2. Physical Fitness. I confirm that I am physically able to participate in my chosen distance. I have consulted, or have had the opportunity to consult, a physician regarding my participation. I will stop participating if I feel unwell or unsafe.

3. Personal Responsibility for Safety. I understand that this is a community fundraising event and not a supervised House program. House does not provide supervision, childcare, or child safety measures for participants at this Event. I am responsible for my own safety and well-being, including staying aware of my surroundings, following course directions, and obeying all traffic laws and posted instructions.

4. Minors. Participants under 18 must have a parent or legal guardian complete and sign this form. A parent, legal guardian, or responsible adult designated by the parent/guardian must accompany and supervise any minor participant for the entire Event. The parent/guardian accepts full responsibility for the minor’s safety and supervision.

5. Assumption of Risk. I knowingly and voluntarily assume all risks associated with participating in the Event, whether known or unknown, even if arising from the negligence of the Released Parties named below.

6. Release of Liability. To the fullest extent permitted by law, I release, waive, and agree not to sue House, its board members, officers, staff, volunteers, sponsors, and Crossroads Community Church (together, “the Released Parties”) for any injury, illness, loss, or damage to person or property arising from my participation in the Event.

7. Medical Treatment. If I am injured or become ill during the Event, I authorize Event staff or volunteers to seek emergency medical treatment on my behalf (or on behalf of my minor child). I understand I am responsible for any resulting medical costs.

8. Photo and Media Release. I grant House permission to use photographs and video taken at the Event that may include me (or my minor child) for promotional and fundraising purposes, without compensation.

9. Acknowledgment. I have read this agreement, understand it, and sign it voluntarily. I understand that by signing, I am giving up certain legal rights.
