What we collect your name and the name you want shown publicly; your professional type or types, practice or organization name, and professional biography; your contact details, city, state, and website or profile link; an optional photo; whether you hold a state license and, if you do, your license types, number, states, and expiration date, your malpractice insurance certificate, and your resume or CV; if you don't, your training, certifications, years of practice, and two references; the services you offer, your areas of specialization, the age groups you serve, the languages you can hold sessions in, the payment options you accept, your formats, the states or locations you serve, and whether you're accepting new clients; your best days and times for sessions; and a short written answer about your experience with culturally responsive care. This form doesn't ask for banking or tax information.

Who can see it only our Executive Director and our Clinical Director.
Shared? Your name, practice or organization name, photo, credentials, professional biography, services, areas of specialization, age groups, languages, payment options, formats, the states or locations you serve, and whether you're accepting new clients appear in the Directory only if you agree below. We don't show your best days and times publicly; we use them to match you with clients. When we match you with a client, we share only the details that client agreed to share.
