Behavior Coaching Client Application

The first step toward helping you and your dog is understanding what's happening and creating a practical path forward.

During this in-home assessment, we'll discuss your dog's behavior, daily routine, history, and your goals. I'll observe your dog in their home environment when appropriate, identify areas to focus on, and determine which coaching approach may be the best fit.

Before your appointment: You'll receive a behavior questionnaire (via email & after this form) to complete in advance. This helps me understand your dog's needs and prepare for our meeting.

Following the assessment, I'll recommend appropriate next steps and a coaching program if we're a good fit.

Assessment Fee: $175

Your $175 assessment fee will be credited toward a recommended coaching program if you enroll within 72 hours of your completed assessment.

There is no obligation to enroll in a coaching program.

The assessment will take place at home unless the information in the questionnaire indicates that a safe public place, such as a park, would be more appropriate. If we suggest a change to the assessment setup or format, we will be in touch.
