F11 Futures — Participation Waiver & Release
Program: F11 Futures Spring College-Prep Soccer Program (high school female players)
Director: Dale Bacino, Blythe, GA
Please read this document carefully. It describes the risks of taking part in F11 Futures and asks you to give up certain rights. A parent or legal guardian must sign for any player under 18, and the player signs too.
1. Program Activities
By signing, I understand the player (“Player”) may take part in any or all of the following (together, the “Activities”):
• Player evaluations on December 13, 2026 at Augusta Arsenal (Georgia) and December 14, 2026 at Palmetto FC (South Carolina), hosted at partner club facilities;
• Four (4) training sessions;
• Six to eight (6–8) scrimmages against college teams;
• Travel to and from games, training, and evaluations, including travel across state lines between Georgia and South Carolina;
• Related warm-ups, team meetings, meals, and other program events.
2. Assumption of Risk
Soccer is a physical, contact sport. I understand that taking part in the Activities carries risks that cannot be fully removed, even when everyone is careful. These risks include, but are not limited to:
• Sprains, strains, broken bones, torn ligaments (including ACL injuries), cuts, and bruises;
• Collisions with other players, goalposts, the ball, or the ground;
• Heat illness, dehydration, cold-weather exposure, and lightning or other weather hazards;
• Uneven or wet field surfaces and facility conditions;
• Playing against older, bigger, and more experienced college athletes;
• Risks of travel, including traffic accidents;
• In rare cases, serious injury, permanent disability, or death.
Concussion and head injury. I understand that heading the ball and collisions can cause concussions and other head injuries, which can be serious and sometimes have long-term effects. I have reviewed concussion signs and symptoms (such as headache, dizziness, confusion, nausea, blurred vision, or memory problems). I agree that:
• The Player will tell a coach right away about any blow to the head or concussion symptoms;
• A Player suspected of having a concussion will be removed from play and will not return the same day;
• The Player will not return to activity until cleared in writing by a licensed health care provider, consistent with Georgia and South Carolina youth concussion laws.
Knowing these risks, I voluntarily choose to allow the Player to take part and I accept these risks.
3. Release and Hold Harmless
To the fullest extent allowed by law, I, on behalf of myself, the Player, and our heirs and family, release and agree not to sue, and to hold harmless, the following (the “Released Parties”):
• F11 Futures;
• Dale Bacino, Director;
• F11 Futures coaches, staff, and volunteers;
• Partner clubs Augusta Arsenal and Palmetto FC, and their officers, staff, coaches, and facility owners/operators;
from claims for injury, illness, loss, or damage arising from the Player’s participation in the Activities, including travel, except to the extent caused by a Released Party’s gross negligence or willful misconduct. I also agree to be responsible for any loss caused by the Player’s own intentional misconduct.
4. Medical Treatment Authorization
If the Player is injured or becomes ill and I cannot be reached right away, I authorize F11 Futures staff to seek and consent to emergency medical care, including first aid, ambulance transport, and treatment by licensed medical professionals. I agree to be responsible for the cost of that care.
Emergency contact (other than parent/guardian): Name: ______ Relationship: ______
Phone: ______ Alternate phone: ______
Medical conditions, allergies, medications: ______
5. Player Health and Insurance
• To my knowledge, the Player is in good health and physically able to take part. I will tell F11 Futures about any medical condition, injury, or limitation, and about any changes during the program.
• I understand F11 Futures does not provide medical or accident insurance for players. I confirm the Player is covered by health insurance, or I accept full responsibility for medical costs.
Insurance provider: ______ Policy/member #: ______
6. Photo and Video Media Release
F11 Futures may take photos and video during the Activities. I give permission for F11 Futures to use the Player’s name, image, and likeness in photos, video, and highlight clips for program promotion, recruiting materials, websites, and social media, without payment.
☐ Opt out: I do NOT give permission to use the Player’s photo or video for promotional purposes.
7. Code of Conduct
The Player agrees to:
• Show respect to coaches, teammates, opponents, referees, college staff, and host facilities;
• Arrive on time, prepared, and with proper gear (including shin guards);
• Follow all coach and staff instructions, especially about safety and travel;
• Not use alcohol, tobacco, vaping products, or illegal drugs at any program activity;
• Avoid bullying, harassment, abusive language, or unsportsmanlike behavior, in person or online.
Parents/guardians agree to model good sportsmanship. F11 Futures may remove a Player who breaks this code, without a refund.
8. Fee and Refund Policy
• The program fee is $100 USD, paid in full at registration.
• The fee is refundable only if the registration is canceled before January 1, 2027.
• No refunds will be given on or after January 1, 2027, including for missed sessions, injury, removal under the code of conduct, or weather.
