How your information will be used

C tutor J Foundation is responsible for this information. Authorised BANGworx staff administer it for the Foundation. Private interviews and records are limited to Sibusiso Molefe and Zimkitha Ndyaluvana. We use your answers to arrange project participation and report combined participation figures. Supporters receive totals, not your individual answers. Optional questions may be skipped without affecting your expression of interest. Race and disability answers need your separate permission. No ID number, document, medical diagnosis or bank details are requested. This is not permission for advertising or recording. To ask for access, correction or withdrawal, contact 072 836 2518 or info@cj-f.org. The Foundation’s physical address is CJF-Community Resource Center, Sada Community Hall, Sada Unit 1, Whittlesea, 5360. We will review unconverted interest records after twelve months and delete or de-identify records no longer needed. Active participant records will follow a separately approved retention schedule. You may complain to the Information Regulator at https://inforegulator.org.za/complaints/. We collect your answers through Jotform. This account stores form data in the United States. An authorised copy is kept in the project’s restricted database hosted by Axxess. These service providers process the information to operate the collection and storage services.
