Sadie Adoption Form

All About Sadie :
Sadie absolutely LOVES other dogs and cats and she really enjoys being around children and people in general. She is the sweetest, most loveable little girl who would really thrive from having an active household with plenty of space for her to play and explore. She is a very quick learner and has a few basic commands down already but is your typical puppy and is into EVERYTHING so mental stimulation and physical exercise is really important to her (a given with her breed mix). She is so playful and just such a lovely dog who is more than ready to find her forever family and give them all the love she has to give.

I give my consent for Midlands Cleft Rescue to gather, process / store my personal data and use my personal data in decision making relating to adoption of a dog. I understand that I may withdraw my consent at any point.
The information I have supplied in this application is accurate to the best of my knowledge.
All pups leave fully health checked by our vet, with minimum first vaccinations, microchipped and a neutering contract in place. Adoption fee is £650.
