MODEL CONSENT & LIABILITY RELEASE: I voluntarily agree to participate as a hair model at The Rainbow Convention. I understand that hair services carry risks, including allergic reactions, irritation, burns, breakage, and results that differ from expectations. I agree to disclose my hair history, known allergies, and relevant sensitivities, and to approve the planned services with my educator before they begin. To the fullest extent permitted by law, I release The Rainbow Convention, its organizers, and participating educators from claims arising from my participation, including ordinary negligence. This release does not cover gross negligence, reckless or intentional misconduct, or liability that cannot legally be waived.
