  • Safety Concern Reporting Form

    YMCA of the Twin Tiers
  • Your voice helps keep our Y safe.

Employees, volunteers, parents and caregivers, members, and youth are encouraged to report anything that may affect someone’s safety or well-being.

You can report suspected abuse or neglect, inappropriate touching or comments, bullying, unsafe supervision, concerning messages, violations of personal boundaries, or anything that makes you feel unsafe or uncomfortable. Concerns may involve an adult, another youth, or someone outside the YMCA.

You do not need proof or know which rule may have been broken. Tell us what you know, in your own words. It is okay to be unsure or leave questions blank.
If someone is in immediate danger or needs emergency medical help, call 911. This form is not an emergency service.

This form does not replace required reports to child protective services, law enforcement, or licensing authorities. Employees and volunteers must follow applicable reporting laws and YMCA procedures without waiting for this form to be reviewed.

    Privacy and protection from retaliation
You may submit this form without giving your name. Providing contact information can help us follow up, but it is optional.

We will handle your report as privately as possible. We cannot promise complete confidentiality. Information may need to be shared with people responsible for protecting individuals, reviewing the concern, or responding as required by law.
The YMCA prohibits retaliation against anyone who raises a concern in good faith or participates in a review. You can report a concern even if it is later not confirmed. Please report any threats, pressure, or unfair treatment related to making a report.

If you prefer to use the anonymous Praesidium Helpline, please call 855-347-0751.
