I am aware of the inherient dangers of participation in the Fall Dance (held on November 19, 2026) and related activities at this event. I will not to hold FAITH HOLISTIC ACADEMY, its officers, agents, employees, or Marion Oaks Community Center liable or pursue legal action against them. This waiver absolves the company of any duty for injuries sustained on the premises before to, during, or after the activity. By signing this agreement, I agree to hold the company completely harmless, including financial responsibility for any injuries sustained, regardless of the cause or circumstances.

We will not offer this dance if we do not have a total of 25 students in attendance.
