I acknowledge that I have received, reviewed, and understand Heart to Home Homecare Service's Awake Overnight / No Sleep Policy. I understand that when I accept an Awake Overnight assignment, I am required to remain awake, alert, attentive, and available to the participant for my entire scheduled shift.

I understand that sleeping or napping while assigned to an Awake Overnight shift is prohibited. I am responsible for following the participant's Plan of Care, completing authorized duties, accurately documenting services in HHAeExchange, and promptly reporting changes in condition, incidents, emergencies, or other concerns according to agency procedure.

I understand that the HHAeExchange Overnight Awake Certification is an end-of-shift certification. It is electronically date- and time-stamped and may not be completed or submitted before my Awake Overnight shift has ended. I understand that I must complete the full shift and all applicable documentation before submitting the certification.

I understand that I may document only duties and services that I actually performed. I understand that falsely certifying that I remained awake, documenting services that were not performed, pre-completing the certification, or otherwise falsifying EVV or service documentation is prohibited.

I understand that Heart to Home may review HHAeExchange records and other appropriate information to monitor compliance. Failure to follow this policy may result in retraining, corrective action, removal from overnight assignments, or disciplinary action up to and including termination, depending on the circumstances.

Employee acknowledgment: My signature confirms my understanding of the policy and my responsibility to comply with it. It does not replace the required HHAeExchange end-of-shift certification for each Awake Overnight shift.
