Martha's Table Survey

Martha’s Table is gathering feedback to help shape its next strategic plan. This survey asks about your experiences, priorities, and hopes for the organization’s future and takes approximately 10 minutes.

Participation is voluntary. You may skip any question or stop before submitting. Your decision to participate—or not participate—will not affect your access to Martha’s Table’s programs or services, employment, or relationship with the organization.

Responses will be stored securely and accessed only by authorized Martha’s Table staff and strategic planning consultants responsible for reviewing and analyzing feedback. Findings will be reported in summary form. Any quotes used will have identifying details removed. Please do not include names or other identifying information in written answers.

Survey data will be retained for 5 years then securely deleted according to Martha’s Table’s data retention policy.
