• Semi-Annual Chapter Health and Success Form

  • This form serves two purposes:

    1. Assist the Awards, Scholarships, & Emblems Committee and National Staff in evaluating Chapter awards
    2. Provide National Council with a snapshot of overall Chapter health beyond membership numbers

    Rather than documenting every activity, Chapters should highlight their most meaningful accomplishments and provide evidence demonstrating their impact. It can be helpful to have various chairs and officers take on specific aspects of the form and for one officer to then bring all the responses together for this final form.
