By registering for and participating in Breakfast/Lunch with Santa, I acknowledge that participation is voluntary and includes eating and drinking, visiting and taking pictures with Santa, holiday crafts, and related event activities.
WARNING OF RISK AND ASSUMPTION OF RISK
I understand that participation involves risks, including slips, trips, falls, contact with other participants, injuries from craft supplies or small objects, choking, burns from hot food or beverages, allergic reactions, and foodborne illness. These risks may result in injury, illness, property damage, or, in severe cases, death.
I understand that reasonable safety precautions cannot eliminate every risk. To the fullest extent permitted by Illinois law, I voluntarily assume the risks associated with my participation and the participation of my minor child/ward named below, including risks arising from the ordinary negligence of Summit Park District, its commissioners, officers, employees, agents, and volunteers.
WAIVER AND RELEASE OF CLAIMS
In consideration of being permitted to participate in this event, I, for myself and, to the extent permitted by Illinois law, on behalf of my minor child/ward named below, waive and release claims for injury, illness, death, property damage, or loss arising out of participation in this event and its related activities against Summit Park District, its commissioners, officers, employees, agents, and volunteers, including claims arising from their ordinary negligence.
This waiver does not release claims arising from willful and wanton misconduct or any liability that cannot lawfully be waived.
FOOD ALLERGIES AND CHILD SUPERVISION
I agree to notify District staff before the event of any known food allergies or dietary concerns affecting me or my minor child/ward. I understand that providing this information does not guarantee an allergen-free meal or environment. I will ask staff about ingredients before consuming or allowing my child/ward to consume food when an allergy concern exists.
I understand that this is a family event. A parent or responsible adult must remain with and supervise minor children throughout the event, including during meals, visits with Santa, and craft activities. I agree to follow event rules and staff safety instructions.
PHOTO AND VIDEO PERMISSION
I authorize Summit Park District to photograph or record me and/or my minor child(ren) listed on this registration during Breakfast/Lunch with Santa. I grant permission for these images and recordings to be used in District promotional materials, including its website, social media, brochures, flyers, and other publications, without compensation.
ACKNOWLEDGMENT
I have read and understand this waiver, including the risks described and the rights I am releasing. I sign voluntarily. If signing for a minor, I certify that I am that child’s parent or legal guardian and authorize the child’s participation.
