ALTERED COURSE PARTICIPATION & LIABILITY AGREEMENT
By registering for or attending any program, class, camp, birthday party, private event, or activity with Altered Course LLC (Altered Course Venue), I acknowledge and agree to the following for myself and for the child named on this form.
ACTIVITIES
Activities may include arts and crafts, painting, slime and sensory play, food and party treats, games, music, dance, theater, and physical activities such as parkour, karate, obstacle courses, running, jumping, climbing, balancing, and tumbling.
ASSUMPTION OF RISK
I understand that these activities carry inherent risks. Physical activities can result in injuries such as sprains, strains, broken bones, or head injuries, including from falls or contact with equipment, the floor, walls, or other participants. Art and craft activities may involve scissors, craft tools, glue, hot glue guns, paint, glitter, slime ingredients, and other materials that can cause cuts, minor burns, skin or eye irritation, stained clothing, or allergic reactions. Food and treats, including items brought by a party host, may contain allergens. Other risks include slips, trips, and falls.
I voluntarily assume all risks associated with participation for myself or my child.
PHYSICAL READINESS
I confirm that my child is able to take part in the activities and I will tell staff about any condition or limitation before the activity begins.
RELEASE OF LIABILITY
I release and hold harmless Altered Course LLC, its owners, staff, instructors, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of participation in or attendance at any program, class, camp, party, event, or activity, including those caused by negligence, to the fullest extent permitted by law.
BEHAVIOR & SAFETY EXPECTATIONS
Respectful and safe behavior is required at all times. Participants must follow all staff instructions and safety guidelines. Disruptive, unsafe, or inappropriate behavior may result in removal from a program, class, party, or event without refund.
HEALTH, ALLERGIES & MEDICAL AUTHORIZATION
I will tell Altered Course about any allergies, medical conditions, or special needs on this form, and I will update them if anything changes.
In the event of a medical emergency, I authorize Altered Course LLC staff to seek appropriate medical care for myself or my child. I understand that I am responsible for any medical expenses incurred.
ILLNESS
I will keep my child home with a fever or other signs of illness.
ALCOHOL RESPONSIBILITY (ADULT EVENTS ONLY)
For events where alcohol may be present, I confirm that any participating adult consuming alcohol is 21 years of age or older and assumes full responsibility for their actions. I release Altered Course LLC from any liability related to alcohol consumption.
AGREEMENT & CONSENT
By signing this form, I confirm that:
- I have read and understand this Participation & Liability Agreement.
- I voluntarily accept all risks associated with participation.
- I agree to follow all policies and safety guidelines.
- I consent to participation for myself or my child.
