• Birthday Club Referral Form

  • Sebby's Corner provides basic essentials to families so that no child goes without.

    Children in our Birthday Club receive a gift, balloons, decorations and a birthday cake. Any child turning 10 or under can be referred, if they are from a disadvantaged background. Any professional can refer a child, including teachers, but a family cannot self-refer.

    PLEASE NOTE:
    The birthday referral must be one week in advance of the birthday. If the birthday has passed unfortunately we will not be able to support this time
    We require two week's notice for collection for our team to put the birthday club referral together
    We cannot provide gifts for children whose birthday have passed

    Please note if the child is a twin or triplet you will need to fill out one form per child.

    Due to issues with our building, our Birthday Club collections will NOT be taking place at our hub. Instead, we will be at Workshop C, Unit 3, Panattoni Park, York Crescent, Borehamwood, WD6 1DJ.

    We will use the information you provide in this form for the purposes of providing our services to you. All of the data you provide in this form will be held in line with our privacy policy. Your information will only be shared with third parties where it is necessary in order to provide our services to you.

  • What happens next?


    Once you have submitted this form, our team will review the referral to assess the family's eligibility for Birthday Club. You will receive an email confirming the outcome of the referral, whether it has been approved or declined. If the family is eligible, the email will include a link to book their collection appointment. You can either book the appointment on the family's behalf or forward the link directly to the family so they can choose a suitable date and time themselves.
