Terms and Conditions:
By submitting this form, I acknowledge that participation in dance classes involves physical activity and I hereby release and hold harmless the Madrid Dance Company LLC (also known as "MDC", "Ballet Folklorico Inaquito" and "BFI"), Rosemarie Madrid, her or it's agents and employees in the event that I or my child(ren) suffer injury or any illness or damages while undertaking the course of instruction.
I understand that there are NO REFUNDS of tuition or fees for any missed classes or for classes cancelled due to emergencies beyond MDC's control. If classes are cancelled due to severe weather, safety hazards or power outages, the studio will make every effort to provide a make-up class.
Madrid Dance Company, its staff or affiliated dance centers are not responsible for any lost or damaged personal items.
By registering for our dance classes, you agree to the collection and use of your personal information (such as name, contact details, and payment information) solely for administrative, billing, and instructional purposes. We value your privacy and strictly enforce a policy that your private information will never be shared, sold, rented, or disclosed to any third parties, except as required by law or to process secure financial transactions. All data is securely stored and accessible only by authorized personnel to ensure your safety and confidentiality.
Code of Conduct & Right of Dismissal:
Enrollment is a privilege, not a right. We strive to maintain a positive, safe and uplifting environment for all dancers, families and staff.
• Respectful Behavior: Parents, guardians, dancers and family members are expected to conduct themselves in a cooperative, courteous and respectful manner at all times—whether inside the studio, in waiting areas, at off-site events or online.
• Prohibited Conduct: Disrespectful attitudes, aggressive communication toward staff or students, public or digital gossip, slander or attempts to disrupt studio operations or sow discord will not be tolerated.
• Right to Dismiss: Madrid Dance Company LLC reserves the right to ask any parent, guardian or family member to leave the premises immediately and/or to terminate a dancer’s enrollment without warning if this Code of Conduct is breached.
• Financial Disclaimer: In the event that a student is dismissed due to a breach of conduct by the dancer, parent, or family member, all paid tuition, registration, and performance fees remain non-refundable.
Media Release: You give Madrid Dance Company, LLC permission to take photos, videos, and audio recordings of your child (or you) during their classes, events, and activities. If the person participating is a child under 18, the parent or legal guardian is giving this permission on their behalf. I acknowledge and consent to allow Madrid Dance Company to use photos and/or videos of my child/myself in publications, advertisements, MDC's website and/or on social media sites. I further understand no financial compensation will be provided for use of these photos/videos.
