1. Gather documents to bring in with you to your visit
   1. All up to date Insurance Cards including vision insurance if applicable
   2. A Photo ID
   3. List of all medications including over the counter with dosage information, as well as allergy information.
   4. Know your current height and weight
   5. Referral from Primary Care Physician if needed by your health insurance plan
2. For our Diabetic Patients, we would like you to see your PCP 1^st before your visit with us and obtain A1C and bring a copy with you.
3. Please bring your current pair of glasses and if applicable your box of contact lens that you are currently wearing.
   1. If you are a contact lens wearer, please bring information on the last time you put in the new lenses and the type of solution you use
