COLORS YOGA FOR KIDS WAIVER & LIABILITY RELEASE
I, {parentlegalGuardian}, am the parent/legal guardian of {childsInformation} ({childsDate}). I hereby consent to {childsInformation} participating in kids yoga classes conducted by COLORS Yoga for Kids.
I understand and acknowledge the following:
Assumption of Risk: I am aware that participation in physical activities such as yoga involves certain inherent risks, including but not limited to physical injury. I knowingly and voluntarily assume all such risks.
Supervision and Responsibility: I understand that COLORS Yoga for Kids will provide yoga instruction during the class but is not responsible for supervising my child outside of the yoga activities. I acknowledge that this class is not a babysitting service, and I am solely responsible for supervising my child and ensuring their safety during the class. COLORS Yoga for Kids instructors will not chase after children who leave the group or the designated yoga area.
Behavioral Expectations: I agree that COLORS Yoga for Kids may require my child to leave the class if their behavior is disruptive or poses a risk to themselves or others, and no refund will be provided in such cases.
Photo/Video Release: I grant permission for my child's photograph or video to be taken during the classes for promotional or educational purposes related to the yoga program.
Medical Authorization: In the event of an injury or medical emergency involving my child, I authorize COLORS Yoga for Kids to seek medical treatment for my child if I am not immediately available or unable to consent for any reason.
Waiver and Release: I, on behalf of myself, my child, heirs, executors, administrators, and assigns, hereby waive, release, and discharge COLORS Yoga for Kids, its officers, employees, and agents from any and all claims, liabilities, demands, actions, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or to any property belonging to my child, whether caused by the negligence of COLORS Yoga for Kids or otherwise, while participating in the kids yoga class.
GO PLAY! FACILITY & RECREATIONAL PLAY ACKNOWLEDGMENT
I understand that certain COLORS Yoga for Kids programs include recreational playtime at Go Play!, a separately owned and operated indoor playground.
I acknowledge that recreational play involves inherent risks, including falls, collisions, and other injuries associated with playground equipment and physical activity.
I understand that COLORS Yoga for Kids provides yoga instruction but does not own, operate, maintain, or control Go Play!'s playground equipment or recreational play areas.
I understand that a parent or legal guardian must remain on-site and is responsible for supervising their child during recreational playtime. COLORS Yoga for Kids' instructional supervision responsibilities end when the scheduled yoga class concludes.
I acknowledge that Go Play! may have its own facility rules, participation requirements, and liability waiver, which must be followed as applicable.
I have read this waiver and release of liability, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
