Open Gym Registration – Super Kids Playschool

1. Maximum of 2 children per adult. Every child must be accompanied and actively supervised by a registered adult at all times. 2. The adult must remain in the facility for the entire session and may not leave children unattended, including in washrooms, hallways or the parking lot. 3. All children and adults must use equipment only as directed by Super Kids Playschool staff and as appropriate for their age and ability. No climbing, jumping or flipping on equipment without adult supervision. 4. Participants who are ill, or who have had symptoms of illness within the past 24 hours, must stay home. 5. Clean athletic or dance attire is required. No jewelry, hard accessories or gum. Long hair should be tied back. Bare feet or socks with grips only. Warts must be covered. 6. No food or drink on the gym floor. Water bottles are permitted. 7. Any injury, however minor, must be reported to staff right away. 8. Super Kids Playschool is not responsible for lost or stolen belongings. 9. Super Kids Playschool reserves the right to refuse entry to, or remove, any participant who does not follow the rules or whose behaviour is unsafe or disruptive, without refund. 10. Pre-registration is required, as ratios must be maintained. Registration closes on Monday, October 26. Walk-ins cannot be accommodated.

Acknowledgement of Risk, Waiver of Liability and Indemnity Agreement In consideration for being permitted to use the facilities, programs and services of Super Kids Playschool, the undersigned, on behalf of himself or herself and all participating children and dependent adults listed on this form, as well as any personal representatives, heirs and next of kin (collectively the "Guest"), acknowledges and agrees that: I) The Guest has read, understands and agrees to abide by this Acknowledgement of Risk, Waiver of Liability and Indemnity Agreement, the Open Gym Rules, and all health and safety requirements and rules posted at the facility. This agreement applies to every person registered on this form and to every future Open Gym visit unless a new form is requested. II) The Guest hereby releases, waives, discharges and covenants not to sue Super Kids Playschool and its directors, officers, employees, volunteers and agents (collectively the "Releasee") from all liability to the Guest for any loss or damage, and any claim or demand on account of any property damage or any injury to, or illness or death of, the Guest (including any loss, damage, injury, illness or death arising from communicable illness), whether caused by the negligence (active or passive) of the Releasee or otherwise, while the Guest is in, upon or about the premises of the Releasee, using any facilities or equipment of the Releasee, or participating in any program affiliated with the Releasee. III) The Guest agrees to indemnify and save and hold harmless the Releasee, and each of them, from any loss, liability, damages or costs the Guest may incur, whether caused by the negligence (active or passive) of the Releasee or otherwise, while the Guest is in, upon or about the premises of the Releasee, using any facilities or equipment of the Releasee, or participating in any program affiliated with the Releasee. The Guest understands that the Releasee is not required to provide insurance to cover the Guest in the event of illness, injury, death, property loss, theft or damage of any sort. IV) The Guest acknowledges that use of the premises, facilities, equipment and services of the Releasee may involve inherent dangers and risks, including the risk of physical illness or injury, death or property damage. These risks include falls, collisions with other participants or equipment, and injuries from tumbling, climbing, jumping, balancing, aerial and gymnastics type activities. The Guest assumes full responsibility for these risks, whether or not caused by the negligence (active or passive) of the Releasee. The Guest acknowledges that any illness or injury may be compounded by negligent first aid or emergency response of the Releasee, and waives any claim in respect thereof. V) The adult signing this agreement confirms that they are the parent, legal guardian or authorized caregiver of each child listed on this form, has the authority to sign on their behalf, and accepts full responsibility for supervising those children at all times, with no more than 2 children per adult. The adult also confirms that each participant is in good health and able to take part, and will tell staff about any medical condition, injury or allergy before the session begins. VI) The Guest authorizes first aid treatment at the premises of the Releasee and, in a medical emergency, permits the Releasee to arrange emergency medical services and hospitalization. The Guest is responsible for any related costs. VII) This agreement is intended to be as broad and inclusive as is permitted by the laws of the Province of Alberta. If any portion is held invalid, the balance continues in full force and effect.
