Saint Francis of Assisi Church
The Sacrament of Baptism is celebrated after the last vigil Mass on Saturdays by appointment. Parents must complete this Baptism Registration form and the Baptism Sponsor Eligibility form.

After these are completed, please call the Parish Office at (941) 697-4899 to schedule an appointment with the Pastor. After this interview, the Pastor will schedule the Baptism.
