I, the undersigned parent/guardian of the participant listed on this registration form, understand that participation in DNA Athletics LLC activities, including but not limited to flag football practices, combines, clinics, training sessions, games, camps, and other athletic activities, involves inherent risks.These risks may include, but are not limited to, falls, collisions, concussions, contact with other participants, injuries resulting from athletic activity, equipment-related injuries, weather-related conditions, and other risks associated with participation in organized sports.I voluntarily authorize my child to participate in DNA Athletics LLC programs and acknowledge that participation is chosen freely and voluntarily.In consideration for allowing my child to participate, I agree to release, waive, discharge, and hold harmless DNA Athletics LLC, its owners, members, coaches, instructors, employees, volunteers, representatives, agents, and affiliates from any and all claims, demands, damages, liabilities, expenses, or causes of action arising out of or related to participation in DNA Athletics LLC activities, except for claims resulting from gross negligence or intentional misconduct.I understand that it is my responsibility to ensure my child is physically capable of participating and that I have disclosed all relevant medical information that may impact participation.I acknowledge that my child is expected to follow all rules, instructions, and safety guidelines established by DNA Athletics LLC.By signing below, I confirm that I have read, understand, and voluntarily agree to the terms of this Liability Release and Assumption of Risk Agreement.

I authorize DNA Athletics LLC coaches, staff members, and designated representatives to act on my behalf in the event of a medical emergency involving my child during participation in DNA Athletics LLC activities.I understand that reasonable attempts will be made to contact the parent/guardian or emergency contact listed on the registration form before medical treatment is provided whenever circumstances allow.However, in the event of an emergency where immediate medical attention is required, I authorize DNA Athletics LLC representatives to seek and obtain appropriate medical care, including evaluation, transportation, and emergency treatment as deemed necessary by qualified medical professionals.I understand that DNA Athletics LLC is not responsible for medical expenses incurred as a result of injury or illness during participation. Any medical costs, including ambulance transportation, hospital services, physician services, or other treatment, remain the responsibility of the participant’s parent/guardian and/or insurance provider.I certify that the medical information provided during registration is accurate and complete to the best of my knowledge.

I grant DNA Athletics LLC permission to photograph, record, and capture video of my child during DNA Athletics LLC activities, including but not limited to combines, practices, clinics, training sessions, games, and events.I authorize DNA Athletics LLC to use these photographs, videos, and recordings for promotional, educational, marketing, and informational purposes. This may include use on:DNA Athletics LLC websiteSocial media platformsPromotional materialsDigital advertisementsEvent highlightsTeam and program announcementsI understand that my child’s name, image, likeness, and athletic performance may appear in these materials.I waive any right to inspect or approve finished materials and release DNA Athletics LLC from any claims related to the authorized use of these photographs, videos, or recordings.I understand that participation in DNA Athletics LLC activities requires granting this permission.

I understand that registration for the DNA Turkey Bowl Clinic on November 29th is non-refundable within 7 days of the event (after November 22nd). Cancellations requested prior to November 22nd are subject to a processing fee. In the case of severe weather, event credits will be issued for a rescheduled date or future DNA program.
