Thank you for your interest in adopting from Operation Safe Haven MOV! Please answer these questions honestly. Completing this form does not guarantee adoption. We reserve the right to refuse any application at our discretion. Let it be known that Operation Safe Haven MOV will ensure the animal is fixed, vaccinated, dewormed, flea treated and litter trained before adoption. We do require a $50 adoption fee; this fee goes directly to our medical bills to provide these animals the best care possible. Cash, check, Venmo or cash app is accepted the day of animal adoption.
