Field Trip Consent Form

Please read the following contents of this Consent and Acknowledgement of Risk form and be sure to clarify any questions or concerns BEFORE signing.

CONSENT ACKNOWLEDGEMENT OF RISK FORM

I acknowledge my right to obtain as much information as I require about this program/ activity and associated risks and hazards, including information beyond that provided to me by the school.

1. I freely and voluntarily assume the risks/hazards inherent in the program/activity and understand and acknowledge that my child may suffer personal injury arising from his/her participation.
2. My child has been informed that he/she is to abide by the rules and regulations, including directions and instructions from the teacher(s) over all phases of the activity.
3. In the event my child fails to abide by these rules and regulations, consequential action may require his/her exclusion from further participation or that I be contacted to have him/her picked up, unless I have specified other transport arrangements.
4. I acknowledge that it is my responsibility to advise the teacher of any medical and/or health concerns of my child that may affect his/her participation in the stated program or activity.
5. I acknowledge that the trip supervisors may secure transport to emergency medical services as they deem necessary for my child’s immediate health and safety, and that I shall be financially responsible for such services.

RISKY PLAY LIABILITY WAIVER & RELEASE OF CLAIMS

Assumption of Risk

I understand that my child’s participation in the outdoor education program at Nature Nurtures Academy involves exposure to outdoor environments and natural elements, including hiking, hands-on ecological learning, use of basic outdoor tools under supervision, nature exploration, and physical activity on uneven terrain and in varied weather conditions. These activities carry inherent risks such as but not limited to tripping, insect bites, minor scrapes, sun exposure, or unforeseen natural hazards.

I acknowledge that:

• Program staff will take all reasonable precautions to ensure a safe and supervised environment.
• Some risks are beyond the control of staff and cannot be eliminated entirely.
• My child has been informed that he/she is to abide by the rules and regulations, including directions and instructions from the teacher(s) over all phases of the risky play activity
• In the event my child fails to abide by these rules and regulations, consequential action may require his/her exclusion from further participation.

I voluntarily accept and assume all known and unknown risks associated with my child’s participation.

Release of Liability

In consideration of my child being allowed to participate in Nature Nurtures Academy programs, I, on behalf of myself, my child, and our heirs, executors, and assigns, hereby release, discharge, and hold harmless Nature Nurtures Academy, its staff, volunteers, agents, and affiliates from any and all claims, liabilities, demands, or causes of action arising from injury, illness, loss, or damages that may occur as a result of participation in the program, except in cases of gross negligence or willful misconduct.

Medical Authorization

In the event of an emergency, I authorize staff to secure medical treatment for my child and agree to assume financial responsibility for any associated costs.

CONSENT FOR USE OF PHOTOGRAPHS

I,   First Name*   give Nature Nurtures Tay Forest School permission to use photographs of my child, {childsFull}, on the Nature Nurtures Tay Forest School website and/or social media posts promoting the school. We acknowledge that the photographs will not be used for any other reason unless otherwise specified and a new consent form is signed. The photographs will remain the sole property of the Educators at Nature Nurtures Tay Forest School. At any time, I can request in writing that {childsFull} photograph(s) be removed.
